New Patients Registration Details

The practice accepts patients from within this practice area. If you wish to register please call at reception. You may be asked to provide photographic ID as per BSO requirement. Staff will be happy to guide you through the procedure. We will ask you to complete a health questionnaire which will provide useful information whist we wait on your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any medication you may be taking. Medical treatment is available from the date of registration. Please contact reception for further information.

Registration Forms can be printed below and completed. The completed form should be brought into the Practice as photographic ID and proof of address is required, this is part of the registration process.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Phone First

Phone First is for an injury or illness which requires urgent treatment but is not immediately life threatening. You need to ring phone first before attending CAH/MIU on: 0300 123 3111 | Monday to Friday: 9:00am to 9:00pm | For Weekends & Bank Holidays: Call 028 3756 7481 10:00am to 6:00pm.

Accident & Emergency Departments

Your nearest full time Accident and Emergency department is Craigavon Area Hospital – 028 38334444.

Infectious Disease

If you have a suspected infectious disease, please inform reception if you suspect an infectious disease, as this will enable us to deal with it appropriately during your visit to protect you, other patients and staff.

Chaperone Policy

Patients are encouraged to ask for a chaperone, if required, at the time of booking their appointment wherever possible.
